Dear member,
My son's birth certificate should be corrected with following changes, as he would be appearing for SSC/ 10th in March'14 and would also like to apply for passports:
FYI, my son was born in Hyderabad (Birth Cert., issued by HYD Municipal Corp) and we currently live in Vijayawada. Kindly suggest correct procedure.

you have to give a affidavit with correct name with all the details with the existing birht certificate to the municpal office were you had taken from hyderabad only
if you have supporting documents then give those copies

Thank U sir for your prompt reply.
Kindly mention on how much stamp paper value should the affidavit be made. Also mention how soon the corrected birth certificate would be issued.
