Missmatch in father's name on certificate and address proof

My dad's actual name is Rohit Prasad. But, Since my father did not have his identity proof at the time of my school addmission, he submit affidavit which stated his name as Rohit.

So all my school education states my name as Rohini Rohit Sharma.

But my address proofs like PAN, Aadhar, Bank details has name as Rohini Rohit prasad Sharma.

This mismatch between father's name in both the Proofs are creating problem for me in Passport procedure.

Could you please provide me with solution as Passport office is not providing proper solution to me in this case.

If it is only in your address proof then you need to adopt change of name procedure as per Passport issuing authority rules and regulations.

And if in the event your father's proof also shows name difference then your father need to also adopt change of name procedure as per Passport issuing authority

Well, I want my name as Rohini Rohitprasad Sharma, since my dad's actual name is Rohitprasad.

What would be affidavit mentioning? Would affidavit will show name as "From Rohini Rohit Sharma" "To Rohini Rohitprasad Sharma"? Or my dad will hacve to change name from Rohit to Rohitprasad? If yes, then since my dad's actual name is Rohitprasad, we donot have any document which shows only Rohit.
