There is no set format of this. You can ask the spouse to just write over a simple paper with his/her signature and thumb impression on it.

Keep up the promises are very important for every human life. When we don't keep a promise to someone, it communicates to that person that we don't value him or her. We have chosen to put something else ahead of our commitment. Even when we break small promises, others learn that they cannot count on us

@ Aman Chawla- I mean to say that one of the spouse wants the written declaration in court stamp Paper. In how much Rs stamp this declaration must be considered Valid in this case.

Any declaration made on any amount of non-judicial stamp paper will be of no valid value of the person signing it denies its authenticity. Better it is to engage sensible lawyer to make any agreement get it properly signed, witnessed and registered or notarized so the authenticity remains intact and your wife is unable to deny it at any stage in any proceedings anywhere.

Marriage is a never-obeyed declaration that there shouldn't be extra-marital s*x.     Any further declaration will only be redundant.

@ Vijay - Already discussed with a criminal lawyer. He has told get the declaration done in non judicial stamp paper with signature and thumb impression on it and with few witnesses. After that it can be notarized by notarized Advocate or authorised by any magistrate. Your view on this?
