Joint ownership vs. settlement

Can a property registered in a mother's name be changed to joint ownership with her children? What are the legal steps to be taken? Are there any tax implications?  

After her lifetime can her property be inherited by the children if she writes a registered will?

(1) If it is a registered "Sale Deed", no scope of alterations & additions with regard to ownership rights. (2) If she executes a "Will Deed", the execution rights will be inherited to her children, only after her life time.
