Intermediate petition template required for party in person

Dear Lawyer,

I appear party in person, for a 498a case in Bangalore.   I felt the petition given by my earlier lawyer ( a bank lawyer friend came for help) does not write any incidents on it. so want to describe in detail and want to attach necessary evidence in one shot. 

I need a document template for this, content I believe i am capable to prepare.
