How to add surname

my previous name is "NAVEEN KUMAR" and present name is "NAVEEN GARG",and i wants to appy passport, please tell me what legal documents i need for it(passport at NAVEEN GARG name)

my all educational certificates at old NAVEEN KUMAR name,so how i get PAssport at new name NAVEEN GARG

my birth certificate,uid,ration card all have new name(NAVEEN GARG)

please help

There are specific annexurs as per to the pass port department, you just get solemnly affirmed relevant to you before a magistrate .

For including the surname to your name, you can use the form Annexure-E and attach the same to your passport application. You get the said form which will provide your details about what to do on it and other details.
