Hello, My name is "Anil" in my 10th, B.Sc., M.Sc. and Ph.D certificates. In my 12th certificate, my name is "Anil Chaudhary". My father's name is Ravindra Chaudhary". Now, I want to incorporate my sir name as Jakhar. Would it be appropriate or should I get incoropated as Chaudhary. Secondly, what is the correct and acceptable procedure is incorporation of Sir name like giving changing in name in two national newz papers and then getting order from the court, etc. Well, I need a single document which can state that Anil, Anil Chaudhary and Anil Jakhar are one and same person and now onwards he can be known as Anil Jakhar.
