Change in passport name after divorce

Dear Expert,

I am legally divorce, working at Singapore. I recently applied for a name change in my passport from my married name to my Maiden name. Now Indian commission of Singapore is asking for Affidavit and magistrate copy stating about my name change.

Kindly help me how can i get these documents sitting here. Also, if any specific format for the documents.
