I need help in resolving issue related to my birth certificate (BC).
The issue was raised during applying for passport.
I was born on 14.12.1989 at brahmavar , Upudi taluk of dakshina kannada Dist. karnataka.
Following are the errors present on BC.
1. My name ( Shridhar Shenoy ) is not mentioned in BC instead it is left blank. This issues was resolved by providing a declaration by parents. But I need to enter my name on BC.
2. My mother's name is given as Jayashree ( which was her maiden name before marriage) Whereas all other documents mentions it as Geetha Shenoy ( her name after marriage). I need to change my mother's name on BC to Geetha Shenoy. ( to resolve this issues during my collage admission i had submitted a notary affidavit mentioning both names of my mother in which she has declared that " her name was changed to geetha after marriage but all her previous records says it as jayashree").
As there is no information regarding this is available on any govt sites, Help in this regard is highly appreciated.
