Dear sirs ,
I would appreciate if any one could help me.
I born in 1989 , but my parents did not realize that my mothers name is wrongly mentioned in my birth certificate .Now i need to apply for my passport . Therefore i need to correct my mothers name in my birth certificate , Is there any solution ?

You can apply for correction in the local municipal office with the submission of your affidavit and other formalities.

Thank u sir . I born on 23 Jan 1989 , do i need submit birth certificate for passport application
