Documents to be collected from member of co-op housing society

Dear Sir.
I would like to know the documents to be collected from a member of Co-operative Housing Society for change in the name (Female) after marriage? Is only Marriage certificate enough for changing the name or we have to ask for Gazetted change of name is also necessary to effect the change

Marriage certificate is enough.
