How to apply for passport

Hi,

My wife wants to make a passport for her mother. Her mother does not have a school leaving nor she has a date of birth certificate. She was born in a village so she does not remember anything about documents and she is now 50 years. She misplaced the Nikah Nama (marriage certificate)as well.

Please let us know how can we go about making the passport. Please give us some tips for hassle free process.

There is a provision of submitting an affidavit duly notarized. Please look into the following rule:
4. BIRTH DOCUMENTS

1. Applicants born on or after 26th January, 1989, should produce an attested copy of the birth certificate, from Municipal authorities if born with in territorial jurisdiction of a Municipal authority or otherwise from the office of the Registrar of Births and Deaths.

2. Applicants born before 26th January, 1989 should produce attested, copy of any of the following documents. -

(i) Birth certificate issued by a Municipal Authority or District office of the Registrar of Births and Deaths.

(ii) Date of birth certificate or nationality certificate from the school last attended by the applicant or any other recognised educational institution;

(iii) A certified copy of extract from the concerned District Magistrate/SDM /SDO from voter’s list where the name of the applicant is entered;

(iv) Date of birth certificate issued by the Educational Boards;

(v) Certificate from the Chief Medical Officer on the basis of the entries in register of the Registrar of Births & Deaths;
(vi) No Objection Certificate issued by State Government or Central Government or Government Undertaking who may be asked to certify the nationality of their employees applying for passport;
(vii) Old Passport;

(viii) Notarised affidavit stating nationality and date of birth in case of illiterate person, Annexure “H”

(CAUTION): Date of Birth/Place of birth once included in the Passport will not be changed under any circumstances. In case of error applicant will have to make a fresh application enclosing relevant evidence for obtaining fresh passport with correct date or place of birth.

In the case of your mother-in-law Annexure "H" will be sufficient.

I agree totally with expert Mr. Malipeddi Jagga Rao except to an error in his sub para viii and last line of his opinion, wherein the format of affidavit will be as per anexure A and not annexure H which has been mentioned by oversight.
