False use of mediclaim policy

'A' & 'B' has taken the Mediclaim policy from Bajaj Alianz Insurance company from Mumbai in March 2012. In April 2012 'A' has made fake hospital documents, that his wife has admitted and taken treatment in that hospital. Claim was submitted for Rs. 20,500 at Pune office. In verification, Doctor whose name was shown in the documents, said patient was not under his treatment.
1) Company want to lodge FIR U/S 420,468,471 IPC. is it correct?
2) If yes, where should they lodge FIR, at Mumbai or Pune?
3) Company has not made payment, still it is possible to lodge FIR?

yes company cna lodge FIr against A for cheating , fabrication of documents . it should be lodged in pune as false claim was submitted in pune , fake hospital documents were of Pune .

For making question more clear, added some extra points,
'A' & 'B' has taken the Mediclaim policy from Bajaj Alianz Insurance company from Mumbai in March 2012. In April 2012 'A' has obtained fake documents from hospital, that his wife was admitted in 'Charu Nursing Home' MUMBAI. Claim was submitted for Rs. 20,500 at Pune office. In verification, Doctor whose name was shown in the documents, said patient was not under his treatment.
1) Company want to lodge FIR U/S 420,468,471 IPC. is it correct?
2) If yes, where should they lodge FIR, at Mumbai or Pune?
3) Company has not made payment, still is it possible to lodge FIR?
