Date of birth affidavit

My child was born on 30.01.2014.Now that I haven't registered her birth within 21 days,the municipality people are asking for an affidavit signed by the executive magistrate.Plz mention what to write in the affidavit.Also plz mention what exactly is an affidavit.And stamp paper of what amount should be bought for the same?Thanks in advance..

Generally Municipality ask to affirm an Affidavit on non judicial stamp paper of Rs.10/- before the Judicial Magistrate along with an order from the Magistrate. For draft the same contact with a lawyer.
