Affidavit sworn before a magistrate for dob for pan card

Please help me out regarding the subject.
From where can I get a Affidavit for DOB to apply for PAN card ,

And how much It will cost

Get an affidavit typed on rs10 stamp. Notarise it and then go before city magistrate for his countersign on the same n file it along with the application for pan

In WB if the affdaivt is notarised then the same can not be sworn before the magistrate. gET A FRESH AFFIDAVIT DONE SO FROM magistrate. contact A LOCAL LAWYER.

Yes, don't you have any other DOB proof like driving licence, Adhaar Card etc. If you do not have, follow the advice of expert Mr.Vijyant Nigam and get the afidavit sworn before the magistrate. Please keep in mind that magistrate of any rank and cadre can attest such affidavit.

@Mr.Thangapandian - Sir please do not address the author of the query with the salutation "Sir". Though you are so benign and humble, we have to respect the relationship of query author and answering expert. You might have observed many youngsters though they get good support by experts, they give scant respect in addressing them. If we address them with "Sir", you can image to what extent some of them may stoop down.

contact a local lawyer . he will prepare the affidavit on Rs 100 stamp paper and have it notarised

Try to get birth certificate from municipal authorities / birth and death registration office. That would be more appropriate.

If you can not, approach some typist near the court / magistrate office, get it prepared and sworn before the magistrate.

Why a notarised DOB certificate is required when you have options to obtain the birth extract from the competent authority?, whether the birth was registered or not? If there are any other proof, this affidavit is not required, contact a local notary public for the needful.
